Please supply us by fax with the following documents.

* Application form :(attached / needs to be signed by the director of the company)

* Companies house or Registry of Companies papers showing Certificate of Incorporation

* Vat registration documents from the relevant authority

* A copy of your company letterhead

* Copy of the passport of the director of the company
